Before You Begin

If you are reporting:
  • A fatality
  • A sexual assault
  • Sexual harassment
  • Hearing Loss
  • An injury/incident involving a fisherman

Please be aware: You are using the guest version of the online application, which will timeout after 30 minutes of inactivity. If you close your browser or timeout, the data will not be saved. We recommend you create an online account (signup here) and call us to verify your account so you can submit the secured application and save your data as you enter it.
 
Information you'll need to make your report. Please allow at least 30 to 40 minutes to complete the online application, and have following information ready:
  • You social insurance number
  • Your personal health care number (BC Care card)
  • Your employer's address
  • Your medical provider's address and phone number (if applicable)
  • Your most recent paystub and earnings for the past 12 months (if you've lost time from work)
  • If you're self-employed and have Personal Option Protection (POP), you'll need your account number
  • If your claim involves a third party, you'll also need their contact information
